Provided on this page are links to medical forms that all intercollegiate student-athletes will be required to complete in order practice and compete in NCAA Division II intercollegiate athletics. The layout is designed to help you quickly and easily find the forms that you need. Instructions are provided with each set of forms.
NEW ATHLETES
A new athlete is one that has never participated in intercollegiate athletics at CSUMB. (i.e. Freshmen, Junior College Transfers, Four Year College Transfers, or any other student attending CSUMB that has never competed at the intercollegiate level.)
All new student-athletes are required to undergo a pre-participation physical screening examination before they can practice or play. A date and time will be advised to you in advance, or by arrangement with the athletic training staff. Please come to your examination appropriately dressed (i.e. shorts, t-shirt, tank tops, sports bra, etc.).
Please download the physical packet using the link below. In the attached packet you will find a variety of forms that you will need to read thoroughly, download, print, fill out completely, and sign before reporting to the designated site for your pre-participation physical examination. There is a checklist & directions on pages 1 and 2 of the packet.
Please note: Physical examinations must be done by Team Physician’s in the CSUMB medical network. Physicals from other physicians are not acceptable.

RETURNING ATHLETES
Student-athletes who have participated in intercollegiate athletics at CSUMB in previous years are not required to undergo a physical screening examination. However, returning student-athletes are required to download, complete and return all of the forms attached to the link below. These forms, together with a back & front copy of your current medical insurance care, must be filed with the Athletic Trainer of your team and approved before you may participate in practice or compete in intercollegiate athletics.

PROSPECTIVE ATHLETES
Student-athletes who will be visiting campus for a try-out with a CSUMB intercollegiate team must download, complete and return all the forms attached to the link below prior to participation in the try-out. These forms are in addition to those required by the Compliance Office. All forms should be returned to the Compliance Office.mpete in intercollegiate athletics.
